Referrals are required
Regardless of whether you are using insurance or paying out of pocket, a referral from your doctor is always required. Access an MD referral form here.
If I have not received your doctor’s referral 48 hours prior to your appointment, your appointment may have to be cancelled/rescheduled
You must verify your insurance benefits
If using insurance, please make sure that you have verified your benefits prior to your appointment. I recommend calling the 1-800 number on the back of your insurance card and using this form here as a guide. If insurance declines to cover your visit(s), you are responsible for the full amount billed.
We are currently in network with Aetna, United Healthcare, and Anthem Blue Cross. A superbill can be provided if we are out of network with your insurance. You can submit the superbill to your insurance and the sessions may still be covered, however, payment is due at the time of service.
The intake process
Once you have requested an appointment, you will receive an e-mail link to our patient portal to complete your intake paperwork.
A credit card is required to be on file to hold your appointment. Your card will not be charged unless you cancel less than 48 hours prior to your appointment or if insurance declines coverage. No-shows or late cancellations will result in a $140 charge.
Cancellations
At least 48 hours’ notice is required to cancel or reschedule your appointment. Your credit card will be charged for late cancellations or no shows.
